Voucher Request Process

There are two steps to applying for a Potassium Iodide (KI) Pill Voucher.

1. Determine Eligibility
   

   In this step, you will enter your zip code to determine if your household is eligible to receive a KI Pill Voucher.

2. Submit Household Information
   

   After your household's eligibility has been determined, you'll need to submit your household details to receive your KI Pill Voucher. Your KI Pill Voucher will be generated upon submission.

Once you have received your KI Pill Voucher, you'll need to take it to a participating pharmacy to receive your KI Pills.
